RESUMO

Acute kidney injury (AKI) is one of the most severe complications of SARS-CoV-2 infection. In a retrospective study, we aimed to describe the influence of COVID-19-related factors on the severity, outcome and timing of AKI in 268 patients admitted in two large COVID-19-designated university hospitals over a period of 6 months. In the univariate analysis, there was a significant relationship between KDIGO stage and the extension of COVID-19 pneumonia on computed tomography (CT), need for oxygen supplementation, serum levels of ferritin, interleukin-6, and procalcitonin, but none of these variables had a value for predicting KDIGO stage in multinomial regression. The odds of recovery of renal function were significantly diminished by d-dimer values. Lack of immunomodulatory treatment was found to be correlated with increased need for renal replacement therapy (RRT). Compared with AKI at admission, hospital-acquired AKI was predicted by the severity of lung damage on CT, evolved more frequently with incomplete recovery of renal function, and was significantly associated with antiviral therapy.

RESUMO

We present the case of a 70 years old female patient with bilateral senile cataract, more advanced at the RE; Primary open angle glaucoma, poor compensation with drugs at the RE (with timolol 0.5% and xalatan 0.005%). Combined cataract and glaucoma surgery was performed, using different surgical approach zones. The article describes the therapeutic options in the morbid association of cataract and glaucoma, pointing out the advantages and the indications for each method.

RESUMO

UNLABELLED: The purpose of this paper is to appreciate the Acrysof Natural IOL comportment during the implantation after phacoemulsification, and to evaluate the visual acuity at 2 months and 2 years postoperative. MATERIAL AND METHODS: Prospective clinical study which comprises 48 patients( 48 eyes) with cataract operated on with phacoemulsification and implantation of Acrysof Natural IOL with the Monarch II injector and the implantation forceps. RESULTS: The patients' age was between 4- 82 years; The IOLs were introduced with the Monarch II injector in 47 cases and with the implantation forceps in 1 case; No IOL was damaged during the implantation; The unfolding and the rotation of the IOL were possible in all cases, without incidents; The visual acuity appreciated 2 months and 2 years postoperative was good in the most of the cases; the chromophore pigment did not influence the quality of vision and the color perception. CONCLUSIONS: The implantation of the Acrysof Natural IOL is identical with the implantation of the Acrysof single piece IOL, is easy to perform and completely atraumatic.

The purpose of the study is to present the favorable results of YAG: Nd laser capsulotomy for the treatment of secondary opacification of the posterior capsule, in our department. A 3 years retrospective study was performed. For 1440 cataract operations, most of them extracapsular extractions with PMMA IOL placed in the posterior chamber (ciliary sulcus or capsular bag), 285 laser capsulotomies were done. The results are presented statistically, including sex and age distribution, type of cataract surgery and implant, treatment protocol (number of sessions, number of impulses per session, energy per impulse) and complications. The frequency of secondary capsular opacification in our study is 19.78%. It is most important in the age group 60-70 years, where the surgery for cataract is performed more often. Capsulotomy was done after extracapsular extraction with IOL placed in the posterior chamber, but especially after extracapsular extraction without implant placement or with complications that have permitted only the use of an anterior chamber implant. Capsulotomy is performed in only one session in 88% of the cases. The energy per impulse is usually 3-5 mJ and the mean number of impulses per session is 10-30. There are few complications: transient elevation of intraocular pressure in 14.73%, discrete laser marks on the implant in 8.77% and moderate iritis in 1% of the cases. YAG: Nd laser capsulotomy appears to be a safe and efficient method of treating secondary cataract; it improved visual acuity in all cases, if different ocular pathology had not influenced it.
